Overview
Adobe Portable Document Format (PDF)
fill-in forms use the features provided with Acrobat 3.0 and later products.
Currently, there is no validation of the information you enter on these
forms. You are still responsible for entering all required information.
The instructions may require some information to be handwritten on the
form after printing, e.g. signatures.
Software Requirements
To view, complete and print PDF fill-in forms you'll need the freely
available Acrobat
Reader (3.0.1 or later) software installed on your computer. Acrobat
Reader does not allow you to save your completed forms to disk.
The ability to save completed forms is available commercially with the
Adobe
Acrobat (3.0 or later) product suite. Form entries are cached,
however, and stay in memory as long as the Acrobat program or plug-in is
running, even if the underlying page is reloaded from the server.
Completing the Form

Download the form by following the links on the Fill-in Forms selection
page. Your web browser may be configured with an Acrobat plug-in to automatically
open the file within your browsers window upon download. You can
fill-in the form within your browser or download the page directly to your
hard disk. (To download the file directly to disk, right click on
the Results Page form title link then select "Save Target/Link As.." when
presented with a menu. Use Acrobat to open the file if necessary.)

Select the hand tool from the Acrobat toolbar menu. You can use the hand
tool to move the page around so that you can view all the areas on it.
A scroll bar on the right hand side of the window also allows you to easily
move up and down the page. Resizing the page within your viewing
window is accomplished with the magnification toolbar and the automatic
page sizing icons.

Position the hand pointer inside a blue form field and click. The I-beam
pointer allows you to type text. The hand pointer also allows you to select
a field, a check box, a radio button, or an item from a list.

Press Tab to accept the field change and go to the next field.

Press Shift + Tab to accept the field change and go to the previous field.
Printing the Form

Use your mouse to select an area of the form that is not inside a form
field before printing your form. If a form field is active (contains the
blinking bar) the contents of that field may not print. Printing
on older computers is a slow background task with version 5.0 (no hourglass).
Be patient.

If the fill-in form is displayed within your web browsers window, it is
recommended you use the printer button on the Acrobat toolbar menu to print
the form instead of your web browsers print function.
